Referrals & placements

A straightforward route into supported accommodation.

We welcome referrals from local authority housing teams, health professionals, probation and voluntary organisations — as well as enquiries from individuals themselves.

Who can refer

If you are supporting someone who needs a stable home

You do not need a completed statutory assessment to start the conversation. A short summary of the situation is enough for us to say whether we can help.

Local authorities

Housing officers, social workers, leaving-care teams and commissioned support services.

Health & justice partners

Community mental health teams, hospital discharge staff, probation and substance recovery services.

Voluntary sector & self-referral

Charities, advice services, family members and advocates — and individuals referring themselves.

What helps a referral move

The information that makes a decision possible

We assess suitability on the basis of need, risk and whether our model of support fits. The more we understand up front, the faster and more honest our answer will be.

  • Basic details — name, date of birth, current location and contact route.
  • Current situation — why the person needs to move, and any deadline involved.
  • Support needs — what help is needed day to day, and what the person can do independently.
  • Risk information — anything relevant to keeping the person and others safe, including history where it affects placement.
  • Funding position — whether a support package or housing costs are in place, and who is funding them.
  • Consent — confirmation that the person knows a referral is being made and agrees to it.

Sending sensitive information? The form below opens WhatsApp, so please share only what is needed to start the conversation. Fuller documents — assessments, risk plans, care packages — can be emailed securely to info@livesafehousing.co.uk.

Move in

Sign-up, induction to the property, and the first key work session booked before the week is out.
